Any coughing or sneezing?
Any itching/scratching, lumps/bumps that you have noticed?
Any changes in appetite, water intake, urinating or bowel movements?
For cats: Is your cat using litterbox or are they urinating or defecating outside the litter box?
If yes: When did you first notice and does pet seem irritated or uncomfortable due to it?
For cats: How many litterboxes are in the house and their locations?
What does your pet eat from the moment they wake up till they go to bed at night? (Please include amount of food given (1/2-1 cup, ¼ can, etc) and number of treats and snacks)
Have you noticed anything unusual in activity level or ability to perform normal activities?
For dogs how often does your pet interact with other dogs? (Dog park, groomers, doggy daycare, boarding kennel)
For cats how often is your pet outdoors or strictly indoors?
Will your pet be traveling, and if so, where?
What supplements or medications has/is your pet on? (Including parasite prevention)
What are your goals for the appointment today?

Would you like Dewormer or Flea and Tick Product today? What Product have you used before and are you happy with continuing to use this product or would you like information on alternate options?
How is housetraining/obedience/create training going? Any questions or concerns?
